Please see the attached flyer for information below on fall cheer tryouts: Fall cheer tryout dates are 5/12, 5/15, and 5/16. Please see the updated attached flyer with information for fall cheer tryouts. They will be held at Tri-North Middle School in the Performance Center. Sports physicals need to be uploaded onto FinalForms and cannot be turned in to the Tri-North front office. It’s also important that your physician complete the IHSAA sports physical form which is attached. Please complete each section of FinalForms and be sure to click ‘Submit Form’ before moving on. When it asks for your student’s email, make sure it is one they can sign in to, as FinalForms will send them a link to confirm their account and sign virtual documents. FinalForms will also ask you to upload a picture of the sports physical. You only need to upload the page titled "Physical Examination Form." You can upload a photo by using a mobile device. Alternatively, you can take a picture with one of those devices, email it to yourself, and then upload it from a desktop or laptop. *Sports physicals completed on or after April 1, 2024, are valid through June 20, 2025. *Sports physicals completed on or after April 1, 2025, are valid through June 20, 2026.
